Working Together for Health and Wellness
The Barrie and Community Family Health Team is one of 200 Family Health Teams (FHT) that were created as part of the Ontario government's plan to reduce wait times and improve access to quality health care for Ontario residents. Since opening our doors in 2006, we have strived to enhance the care that is provided to patients and families within our community by offering a variety of programs and services that support healthy living and overall wellness.
We are one of the largest FHTs in the province, with 67 physicians and 44 integrated health professionals providing care to our 119,000 patients. Family physicians belonging to our FHT work with our team of skilled healthcare professionals to give the best care possible and assist patients with managing their health. The Barrie and Community Family Health Team is Canada’s largest integrated network on one electronic medical record system. Our connectivity within the community ensures we are meeting local health and community needs.
Programs focus on chronic disease management, disease prevention and health promotion, and we work closely with other local organizations, such as Georgian College, Royal Victoria Regional Health Centre, Simcoe Muskoka District Health Unit and CCAC.
